Services

Evaluations

• 15 - minute phone call to answer questions and schedule an evaluation date.

• Intake paperwork and consent forms will be sent. ALL paperwork and consent forms must be completed prior to the evaluation. 

• Written summary will be sent after the evaluation. 

• Individualized treatment plan with long term and short term goals are included with the summary if services are recommended.

• 15 - minute phone call after the report is received to answer any questions and schedule services.

• Progress notes are provided to insurance companies as required.

• Re - Evaluation occurs annually.

• If you just had a recent evaluation completed, bring it with you. It will be reviewed and an informal assessment will be conducted with parent interview.

Therapy Sessions

• Occupational Therapy sessions are 45 minutes. 

• Adhering to your scheduled therapy time is very important.

• Late arrival - Clinician reserves the right to end the session on time dependent upon schedule.

• Please let us know ASAP if you need to reschedule.

• $75.00 cancellation/no show fee will be assessed if cancellation is less than 24 hours or failure to arrive for scheduled appointment. Refer to Attendance/Cancellation Policy in the intake forms.

Consultations

• Consultations with parents and/or team members outside of scheduled therapy time provided on request - Complementary 15 minutes per month.

• Beyond complementary 15 minutes: $75.00/half hour ($37.50/additional 15 minutes)

• Appearance at Team/IEP Meetings - $150.00/45 minutes, prorated for amount of time in meeting.
